1. What is Avascular Necrosis?
Avascular Necrosis (AVN) of the hip, also known as osteonecrosis, is a condition where the blood supply to the femoral head (ball of the hip joint) is reduced or lost. Without blood flow, bone cells die, the hip weakens, and over time the joint can collapse, causing severe hip pain, stiffness, and arthritis.
1.1. Causes of AVN Hip
Trauma/Injury – Hip fractures or dislocations damaging blood vessels.
Steroid Use – High-dose, long-term corticosteroid treatment.
Excessive Alcohol – Blocks blood flow to the bone.
Medical Disorders – Sickle cell disease, clotting problems, autoimmune disorders.
COVID-19 Infection – Recent reports show that COVID infection and/or its treatment (especially steroids) may trigger AVN in some patients.
Unknown Cause (Idiopathic) – Sometimes the exact reason is not clear.
1.2. Symptoms
Gradual hip or groin pain.
Pain worsens with activity (walking, climbing stairs).
Stiffness and reduced range of movement.
Night/rest pain in advanced cases.
1.3. Diagnosis
X-ray – Detects late-stage AVN.
MRI – Best for early detection.
CT Scan – Detailed bone structure if required.
1.4. Treatment Options
Early Stage AVN
Medications and pain relief.
Walking aids to reduce stress on the hip.
Physiotherapy for mobility.
Core decompression surgery – small holes drilled into bone to restore blood supply.
Advanced Stage AVN
Bone grafting or stem cell/bone marrow concentrate injections.
Osteotomy (realignment surgery).
Hip Replacement Surgery – When the joint collapses, replacement is the best option.

In patients with advanced AVN, the most effective treatment is Total Hip Replacement (THR). The Direct Anterior Approach (DAA) is a modern, minimally invasive technique where the hip is replaced through a small incision at the front of the hip, instead of cutting through major muscles.
